How We Work

Hip and Knee Replacement and Reconstruction located in Chicago and Galewood, IL

Why I built my practice this way:
Care you deserve for the life you are meant to live

The greatest thing I can offer a patient isn't just a perfectly executed surgery — though I pursue that relentlessly. It's the feeling that someone truly has them. From the first phone call to the moment they're back doing what they love, I want every patient to feel cared for in a way they never expected from a medical practice. That runs deep in me: long days in my family's restaurant, where hospitality wasn't a policy but a promise; my Greek heritage, where welcoming someone means giving them your very best, without reservation. It's what drives me every day I walk into the office. For years I ran one of the busiest hip and knee replacement practices in the country, more than 1,100 surgeries a year, and I'm proud of every one. I took great care of people within the boundaries of their insurance. But one truth became impossible to ignore: within Medicare, the rules governing what I'm allowed to do - how much time I can give, how my team is built, what care is permitted to look like — are simply too restrictive for the care I believe patients deserve.

I don't want to take reasonably good care of people. I want to take unreasonably great care of them. That isn't possible inside traditional boundaries, so I stepped outside them. Now every patient gets what the system wouldn't let me give - my time, my access, my attention, and a team hired for one purpose: to take care of you the way I'd want my own family taken care of. It's how we practice medicine now. Every patient. Every time.- Dr. Vasili Karas

How it works and how your coverage works

Subtitle, Subtitle

Everyone is welcome in my practice, and every journey starts the same way: a new patient visit. It doesn't matter what insurance you have — Medicare, private insurance, or anything else — come in, sit down with me, and let's talk about your hip or knee and what you want your life to look like. That visit is billed through your insurance.

If you have commercial insurance, the entire concierge experience is billed through your insurance from top to bottom, with no additional surgical fee: your visits, non-operative care, your surgery, your recovery, all of it. Same surgeon, same team, and same unreasonably high standard of care.

If Medicare is your coverage, here's what I want you to understand clearly: your Medicare benefits continue to work for you every step of the way. Your hospital or surgery center, your anesthesia, your implant, your labs and imaging, your physical therapy, your medications, and even non-operative care — all of it is billed to and covered by Medicare exactly as it always has been.

The one exception is the professional fee for your surgeon and surgical team. The level of surgical expertise, technology, and personal care we deliver — robotic-enabled minimally invasive surgery, personalized planning, direct access to your surgeon, and a team built around you — goes above and beyond what is traditionally available in medicine. Because it goes above and beyond, it also goes above and beyond what Medicare covers for the surgical team. That portion is a single, transparent, flat fee: no surprises, no hidden costs, and my team will walk you through exactly how it works and how to make it work for you.

And if, after we talk, you decide a different direction is right for you, I will personally connect you with one of my trusted partners, with a warm, direct handoff, so you are cared for no matter what you choose. You will never leave my office without a plan.

I want your experience with our practice to be unlike anything you've encountered in medicine. You've trusted me with something deeply personal — your health, your mobility, your life. I don't take that lightly, and I never will.

Tom, 66, shares his experience with Dr. Vasili Karas and the team for his hip replacement. “I’m in the sports field, and I felt like an athlete when I was working with them, the way they paid attention to me. They were on top of things from the minute we spoke about the surgery. Even 6 months later at my follow-up, I really appreciated that because, to me, I felt like they really cared.”

Tom is back to living his active life: walking the golf course, biking, and getting in and out of his car without pain. “It’s changed my life. I want to start hiking again. It’s really providing me freedom.”
